The links below provide access to various forms used by the Division of Animal Control. A brief description of the use of each form is also provided.

Application for Hybrid Canine Permit (308k PDF) – To be completed by owners of hybrid canines as defined by Virginia law. All owners of hybrid canines keeping such animals as companion animals (pets) or breeding same in Washington County must obtain and annually renew Hybrid Canine Permits for each such canine in their possession.

Dog Damage Claim Form – To be completed by owners of livestock killed by feral or stray domesticated dogs seeking compensation for losses from Washington County.
